* Abstract

Accidents are rare events across the transportation industries of Europe and North America.  It is, therefore, important that we learn as much as possible from small numbers of adverse events and near miss incidents.  This depends on the skill and expertise of accident investigators.  Although a large number of previous studies report on novel approaches to accident investigation, very few describe the tools and techniques that investigators themselves would recommend to other investigation agencies.  In contrast, this paper will summarise the findings of a joint study between the European Railway Agency and the University of Glasgow to encourage the exchange of lessons learned from investigations and to encourage best practices in incident investigation across national borders.
